HIPAA Notice of Privacy Practices

This notice describes how health information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our Legal Duty

MLC Insurance Agency is required by law to maintain the privacy of protected health information (PHI) and to provide individuals with notice of our legal duties and privacy practices with respect to PHI. We are required to abide by the terms of the notice currently in effect.

What Is Protected Health Information (PHI)?

Protected Health Information (PHI) includes any individually identifiable health information, such as your name, date of birth, health conditions, prescription information, or insurance details that we collect in connection with insurance-related services.

We may use and disclose your PHI for the following purposes: (1) Treatment – to coordinate care with your healthcare providers; (2) Payment – to assist with insurance billing and enrollment; (3) Health Care Operations – to evaluate and improve the quality of our services; (4) As required by law – to comply with legal obligations, court orders, or government regulations.

Uses and Disclosures Requiring Your Authorization

Other uses and disclosures of your PHI not described in this notice will be made only with your written authorization. You may revoke this authorization at any time by contacting us in writing, except to the extent that we have already taken action in reliance upon it.

Your Rights Regarding Your PHI

You have the following rights with respect to your PHI: (1) Right to Inspect and Copy – you may request access to your PHI; (2) Right to Request an Amendment – you may request corrections to your PHI; (3) Right to an Accounting of Disclosures – you may request a list of certain disclosures we have made; (4) Right to Request Restrictions – you may ask us to limit how we use or disclose your PHI; (5) Right to Request Confidential Communications – you may request that we communicate with you in a specific way.

Safeguarding Your Information

We take the privacy and security of your health information seriously. We use administrative, technical, and physical safeguards to protect your PHI from unauthorized access, use, or disclosure.

Changes to This Notice

We reserve the right to change this notice at any time. We reserve the right to make the revised or changed notice effective for PHI we already have about you as well as any information we receive in the future. We will post a copy of the current notice on our website.

Complaints
